Opening a bank account & KYC

KYC Application Form (Central KYC – Individual)

Official form

The standard KYC form used across banks and financial institutions. Your details are stored in the Central KYC Registry so you can reuse your KYC.

Valid identity documents (officially valid documents): passport, driving licence, proof of possession of Aadhaar, voter ID, NREGA job card signed by a State officer, or letter from the National Population Register. PAN (or Form 97) is also required.

Form 97 – declaration if you do not have a PAN

Official form

Needed for opening an account, cash deposits and certain transactions when you do not have a PAN. Replaced Form 60 from 1 April 2026.

Pradhan Mantri Jan Dhan Yojana (PMJDY) account

Official portal

Zero-balance basic savings account with RuPay card and accident insurance. The same government form is used at every bank.

The account opening form is available in the e-documents section of the website, in Hindi and English.

Account opening forms of public sector banks

Each bank uses its own form, but all follow a common structure: the standard KYC section, the bank's own section, Form 97 (if no PAN), a FATCA/CRS tax-residency declaration and the Nomination Form. You can also open accounts online through video KYC or Aadhaar e-KYC at most banks.

RBI allows a simple self-declaration by registered email, mobile, ATM, net banking, app or letter, without visiting the branch. RBI has not prescribed a format.

Nomination (deposits, lockers, safe custody)

Nomination Form (Banking Companies (Nomination) Rules, 2025)

Official form

One form to make, cancel or change a nomination for bank deposits, articles in safe custody and safety lockers. Up to four nominees: for deposits either simultaneously (with percentage shares totalling 100%) or successively (in order of priority); for lockers and safe custody, successively. Banks may also allow e-nomination.

In force from 1 November 2025. The old forms DA1–DA3, SC1–SC3 and SL1–SL3A are no longer prescribed. Your bank will give you the form at the branch or allow it online.

Claim forms for deceased customers (RBI standard formats)

Official form

RBI prescribes standard claim forms that every bank must provide at branches and on its website: Annex I-A for accounts with a nominee or survivor, and Annex I-B for accounts without a nominee. Locker and safe-custody claims have their own formats.

With a nominee or survivor: claim form, death certificate and ID proof of the nominee are enough. No succession certificate is needed. Banks must settle within 15 days of receiving all documents, or pay compensation. Check the compensation β†’

Tax on interest

Form 121 – no TDS on interest (earlier Form 15G / 15H)

Official form

Submit to your bank at the start of the financial year if your total income is below the taxable limit, so that TDS is not deducted on interest.

Small savings schemes (PPF, Sukanya Samriddhi, SCSS)

Form-1: common application for small savings schemes

Official form

One government form (Government Savings Promotion Rules, 2018) for opening accounts under schemes such as PPF, Sukanya Samriddhi Account and Senior Citizens' Savings Scheme, at banks and post offices.

Loans

Official form

After you fully repay a loan, the bank must return your original property documents and remove registered charges within 30 days, or pay β‚Ή5,000 per day of delay (RBI/2023-24/60).

Most banks accept a simple written request for these. These are sample letters, not official forms. Your bank may ask you to use its own form. Fill in the details, then copy or print. Nothing you type is stored or sent.

Frequently asked questions

Are the old nomination forms DA1, DA2 and DA3 still valid?

No. From 1 November 2025, the Banking Companies (Nomination) Rules, 2025 replaced the 1985 rules. A single Nomination Form now covers making, cancelling and changing a nomination for deposits, articles in safe custody and lockers, with up to four nominees. Banks may also offer e-nomination.

What replaced Form 60 and Forms 15G/15H?

Under the Income-tax Act, 2025 and Income-tax Rules, 2026 (from 1 April 2026), Form 97 replaced Form 60 (declaration when you do not have a PAN), and Form 121 replaced Forms 15G and 15H (declaration for receiving interest without TDS).

Is there one account opening form for all public sector banks?

No single form is accepted by every bank. Each bank issues its own account opening form, but they follow a common structure: a standard Central KYC section, the bank's own section, Form 97 if you have no PAN, a FATCA/CRS declaration and the Nomination Form.

Do I need to visit the branch for re-KYC?

If there is no change in your KYC details, RBI allows a self-declaration through registered email, mobile number, ATM, net banking, mobile app or a letter, without visiting the branch. The bank must give you an acknowledgement.

How long can a bank keep my property documents after I repay a loan?

Under RBI circular RBI/2023-24/60, the bank must release the original property documents and remove registered charges within 30 days of full repayment. For delays caused by the bank, it must pay β‚Ή5,000 for each day of delay.
